What is a transcript?: Official transcripts of a student's academic record are sent to colleges, universities, employers and other institutions upon written request by the student. The transcript is a district transcript, including all coursework completed, or "in-progress," at Skyline College, College of San Mateo and/or CaƱada College. Students may also request "issued-to-student" copies to be sent to themselves or picked up in-person. You must provide photo I.D. at time of pick-up.

Remember: The transcript will be in a sealed envelope and may only be considered official if the seal is unbroken.

How to request a transcript: Students can order transcripts online, in-person, by mail, or fax. The first two transcripts requested by a student are free; thereafter a charge of $5 is imposed. Students may request a RUSH service for an additional $10 each. Rush transcripts will be available within 24 hours.

On the web: Transcripts may be requested electronically using WebSMART. Please note: RUSH service is not available through the WebSMART request form.

By fax, mail or in-person: Complete the Transcript Request Form. Transcript request forms are also available from the Office of Admissions and Records, Building 2, Second Floor, or here (link). The form may be submitted in-person, by mail, or fax to:
