Skyline College Health Services offers virtual and in-person appointments!
Appointments are available when classes are in session. IF THIS IS AN EMERGENCY, call 911 or visit your nearest Emergency Room.
Requesting an Appointment
- Register Here. Your username is your Student ID Number (G#).
- Register with your school email or a personal email account that you have access to.
- Complete these items in the “My Forms” tab:
- HIPAA Notice of Privacy Practice (for the Health Center and Counseling)
- SMCCD Telemedicine (Health Center Consent and HIPAA)
- Health History
- TB Questionnaire -- *only required if you need a TB Test
- To complete your appointment request, please call 650-738-4270 or email skyhealth@smccd.edu

Our Services
Health Services providers are available to confidentially advise students. Services include but not limited to:
- Treatment of a variety of illnesses and injuries
- Program physicals/Health screening
- Medication prescriptions and refills, including birth control
- Immunizations / Vaccinations
- Tuberculosis screenings for employment and transfer purposes
- Laboratory testing, including but not limited to titers and sexually transmitted infection testing
- Nutritional information, referrals to internal and external resources and more
- Free fentanyl testing strips
