Requesting an Appointment

  1. Register Here. Your username is your Student ID Number (G#).
    • Register with your school email or a personal email account that you have access to.
  2. Complete these items in the “My Forms” tab:
    1. HIPAA Notice of Privacy Practice (for the Health Center and Counseling)
    2. SMCCD Telemedicine (Health Center Consent and HIPAA)
    3. Health History 
    4. TB Questionnaire -- *only required if you need a TB Test
  3. To complete your appointment request, please call 650-738-4270 or email skyhealth@smccd.edu
    • When requesting an appointment, please include:
      • Your G# and the day and time you would like your appointment.
      • The reason you'd like to be seen.

Our Services

Health Services providers are available to confidentially advise students. Services include but not limited to:

  • Treatment of a variety of illnesses and injuries
  • Program physicals/Health screening
  • Medication prescriptions and refills, including birth control
  • Immunizations / Vaccinations
  • Tuberculosis screenings for employment and transfer purposes
  • Laboratory testing, including but not limited to titers and sexually transmitted infection testing
  • Nutritional information, referrals to internal and external resources and more
  • Free fentanyl testing strips
